YOU NEED A CM12 OR CM12 BASED ROM WITH THE NEW THEME ENGINE!
If status bar icons aren't themed then you have to REBOOT your phone!!!
If you get Contacts force closes: Apply system theme, re-apply this theme and reboot device!
